How does TOC fatigue translate compared to regular season; i.e. how many PAs do you need for a player to make it through a whole TOC?

Depends on lots of variables:
1) [deleted]
2) How many games will your team play (each round is a best of 5 or 7)?
3) Will you get any rest days in the TOC?
4) What position in the line-up?
5) What is your teams OBP?

In general, I think TOC is similar to early season fatigue.  That is, unless they have over 650PA, you will need to rest them once in a while if you want the at 100%.

I'm quite certain regular season usage has no bearing on TOC fatigue.

Crazyfrancostengel77 is right.  TOCs are completely separate from league play.  League usage has no effect on TOC fatigue.

So griz, if I have a guy that had 800 regular season PAs, his fatigue at TOC game one would be equivalent to what it would be like if he had 110 PAs through 20 regular season games, then 115 PAs through 21 games (toc game 2), 120 through 22 (toc game 3), etc.
